Ok, that's the story: the original version of BM (American? English? I do not remember) had not altitude rules. And it worked perfectly. A later version included altitude (as optional if I remember correctly), but they performed poorly.
The last version of BM I found around (I bought it in Ottawa) was back to original and had not the altitude rules included.
For what concerns Italian versions, I think the first one was without while the second had them (after all you have to justify the need of a second edition, don't you agree?).
